Requirement to qualify as follows:
- All retail stores that have purchased from Soccer Stuff N More will automatically qualify with no minimum order necessary. Previous payment terms will apply.
- New Retail stores that want to open an account are required to place a minimum order of $150.00 and required payment on an opening order is done with Visa / MasterCard, Discover, American Express, or PayPal

- Store Name
- Address
- Phone Number
- Owner Manager Name
- Email Address
- State Resale Certificate
- Store Business Card
